Forms with questions before the appointment

Ask customers questions when they book, such as a shoe size or climbing experience, with consent that keeps its wording and answers that stay private.

How-to guide1 min read

A form adds questions to the Details step of the booking page for the services that use it. Northwilds asks for a shoe size before a boot fitting and for climbing experience before a lesson.

Make a form

  1. Under Forms, choose New form.
  2. Add questions. Each one is a short answer, long answer, number, one choice, several choices, yes or no, date or consent.
  3. Give each question its wording and, if it helps, a line of help text. For choices, add at least two.
  4. Switch on Must answer for the questions customers can't skip.
  5. Choose Save form, then pick the form on the services that should use it.

The preview beside the questions shows how they look on the booking page. When the location writes in more than one language, choose a language above the questions and write them in each; anything left empty shows in the main language.

A consent question is a tick box with the wording you write, such as "I agree that Northwilds may store my fitting notes to help me next time". With Must agree to book on, customers can't book without ticking it. Booking Orbit keeps the exact wording they agreed to, with the time, alongside the appointment, so you can show what they agreed to later.

Sensitive answers

Switch on Sensitive for answers that need extra care, such as health details. They are encrypted when stored and hidden from people with the Viewer role.

Changing a form doesn't change answers already given.
